INSURANCE
INFORMATION
Please remember
the patient, not the insurance company is responsible for payment
of professional services rendered. We normally wait for the insurance
company to pay their portion, but request your portion and any
deductibles be paid promptly by one of the methods mentioned below.
As a courtesy to our patients we will process your claims for
you at no extra charge, but
it is your responsibility to
provide us with your insurance information and any changes as
they occur.
TO OUR PATIENTS: We welcome
you and thank you for selecting our office. We will try to provide
for your dental health needs as thoroughly and efficiently as
possible and just as importantly, as reasonably as possible. In
order in keep our fees as reasonable as possible, we strive to
keep our costs as low as possible. One way to achieve this goal
is to ask for payment at the time of service. Before we begin
treatment, an explanation of the recommended procedures and an
estimate of fees involved will be discussed with you. We have
financing for extensive treatments plans available through AMERICAN
GENERAL and DENTAL FEE PLAN.
The following is our office policy
regarding payment for dental services:
1) Cash, check, VISA, MasterCard
or Discover at the time of treatment.
2) Financing through AMERICAN GENERAL
and DENTAL FEE PLAN.
3) A courtesy discount of 3-5% will
be given for payment in full prior to beginning an extensive course
of treatment.
4) In treatment involving laboratory
work, a percentage of the total fee will be requested before treatment
begins.
5) Accounts not paid in full by the
end of the month will be assessed a billing fee of $2.00 or 1.75%
per month (21% annual), whichever is greater.
6) If you have insurance, we ask
that you provide us with all the necessary information needed
to file your claim. We will calculate your portion of the fee
based on that information and you may pay this portion by any
of the above-mentioned options.
7) There may be a charge for missed
or cancelled appointments without 24 hours notice to the office.
